Most people can access their medical records through their patient portal - the online system your doctor or hospital uses to share information with you.

Look for platforms like:

MyChart (by Epic)
FollowMyHealth
Cerner / Oracle Health
Athenahealth
Allscripts / Veradigm
Or a portal specific to your provider or health system

In your portal, you can usually find:

Visit summaries
Lab and test results
Scans and imaging reports
Doctor’s notes and diagnosis codes

You may also be able to download your records as a PDF or plain text, which you can then add to Inciteful Med.
