Signs, Symptoms, and the Diagnostic Path
At a Glance
Symptoms of partial atrioventricular septal defect (pAVSD) vary based on valve leakage, often causing fast breathing, sweating during feeds, and poor weight gain in babies. An echocardiogram is the gold standard for diagnosing pAVSD and determining when medical intervention is needed.
The journey from a suspected heart murmur to a confirmed diagnosis of partial atrioventricular septal defect (pAVSD) often involves a period of intense observation. Because pAVSD is a structural issue that affects blood flow, the symptoms your child experiences—and when they appear—depend heavily on the “leakiness” of their heart valves and the size of the hole between the chambers.
Why Symptoms Vary by Age
It can be confusing to hear that some children with pAVSD seem perfectly healthy, while others require more immediate care. The timing of symptoms is usually driven by how much extra blood is being pumped to the lungs.
- Infants: Many babies with pAVSD are asymptomatic at first. However, if there is significant leakage in the left valve, the heart has to work much harder. You might notice “work of breathing” symptoms, such as tachypnea (fast breathing), sweating during feeds, or difficulty gaining weight (a condition doctors sometimes refer to as failure to thrive) [1][2][3].
- Older Children: If the defect is not found in infancy, a child might eventually show signs of exercise intolerance. They may tire more easily than their peers or complain of fatigue after physical activity [4][5].
- Adults: When pAVSD remains undiagnosed until adulthood, it often presents as shortness of breath during exertion, heart palpitations, or even atrial fibrillation (an irregular heart rhythm) [1][6][7].
Watchful Waiting: When to Call for Help
During the waiting period before surgery, it is vital to know when to seek medical attention.
Call Your Pediatrician or Cardiologist if your child:
- Is sweating more than usual during normal feedings.
- Takes significantly longer to finish a bottle or stops eating early due to fatigue.
- Is not gaining weight along their expected growth curve.
- Shows a noticeable decrease in normal play energy.
Go to the ER or Call 911 if your child:
- Is breathing extremely fast while resting.
- Shows a blue or gray tint around their lips or fingernails (cyanosis).
- Is unresponsive, extremely lethargic, or cannot be woken for feeds.
- Is grunting with every breath or their chest muscles are severely sinking in (retractions).
The Gold Standard: The Echocardiogram
An echocardiogram (or “echo”) is a specialized ultrasound of the heart and is the primary tool used to diagnose pAVSD [8]. It allows the cardiologist to see the heart’s structure, the direction of blood flow, and the movement of the valves in real-time.
Checklist: What to Look for in the Echo Report
When you receive a copy of your child’s echocardiogram report, the following details should be present:
- Location of the Hole: It must specify an ostium primum ASD, rather than a secundum or sinus venosus ASD [3][9].
- Left Atrioventricular Valve (LAVV) Cleft: The report should note whether there is a “cleft” or split in the valve [10][3].
- Severity of Regurgitation: This describes how much blood is leaking backward through the valve. It is usually graded as mild, moderate, or severe [1][11].
- Ventricular Septum Status: To be considered “partial,” the wall between the lower chambers (the ventricles) must be intact without a ventricular septal defect (VSD) [12][13].
- Right Heart Size: The report should mention if the right side of the heart is enlarged (right heart volume overload) [1][3].

Questions to Ask Your Doctor
Curated prompts to bring to your next appointment.
- 1.What is the exact grade of left atrioventricular valve (LAVV) regurgitation—is it mild, moderate, or severe?
- 2.Does the echocardiogram show any evidence of 'right heart volume overload' or enlargement?
- 3.Did you find any evidence of a ventricular septal defect (VSD), even a small one, or is the ventricular septum completely intact?
- 4.Are there any specific feeding restrictions or techniques we should use to help our baby conserve energy?
