Emergency “Red Flag” Symptoms

Lung Emergencies (Pulmonary Hemorrhage)
When the immune system attacks the small blood vessels in the lungs, it causes diffuse alveolar hemorrhage (bleeding into the air sacs) [3].
- Hemoptysis: Coughing up blood or blood-streaked sputum. (Note that lung bleeding can sometimes occur without a cough, so breathing difficulty alone is an emergency) [4].
- Severe Shortness of Breath: Sudden difficulty breathing, gasping for air, or feeling like you cannot get enough oxygen even while resting [4][5]. Do not wait for a home pulse-oximeter reading to drop, as these devices can be inaccurate.

Kidney and Systemic Emergencies
As the kidneys lose their ability to filter waste and fluid, or if your immune system is dangerously low from treatments, emergencies can happen quickly.
- Neurological Changes: Confusion, extreme drowsiness, seizures, or loss of consciousness [8][9].
- Severe Fluid Overload: Rapidly worsening swelling in the legs, or fluid in the lungs that makes it impossible to lie flat without gasping for breath [10][8].
- Signs of Severe Infection: A high fever, uncontrollable shaking/chills, or feeling extremely weak and dizzy, especially while taking immunosuppressant medications like cyclophosphamide or steroids.
Symptoms That Require Prompt Reporting
Not every symptom requires a 911 call, but when dealing with Anti-GBM disease and powerful medications, you should never wait and see. Contact your specialist or clinic the same day if you notice:
- Changes in Urine Color or Output: A sudden, marked decrease in how much you urinate, or urine that looks dark, “cola-colored,” tea-colored, or visibly bloody (hematuria) [11][12]. Follow your doctor’s exact instructions for what volume of urine is concerning for your specific case.
- New Swelling (Edema): Puffiness in the face (especially around the eyes) or new swelling in the ankles and feet [10].

How the Disease Progresses
Anti-GBM disease does not typically follow a slow, “simmering” course. Instead, it is an acute event. It can worsen rapidly over a period of days to weeks [2].
The disease can be “silent” in its earliest days, with only microscopic amounts of blood in your urine that you cannot see without a lab test [15]. However, the inflammatory process can escalate very quickly [16].
