Recognizing Emergencies and Red Flags

New, severe, worsening, or unusual symptoms in someone with atherosclerosis can signal a heart attack, stroke, sudden loss of blood flow to a limb, or dangerous bleeding. Call emergency services immediately—even if symptoms improve or occur without chest pain.
Living with atherosclerosis means understanding the difference between predictable symptoms of narrowed arteries and unpredictable crises caused by sudden blood clots. While stable symptoms are managed over time, acute events are medical emergencies where time is tissue—the faster you receive care, the more heart muscle, brain cells, or limb tissue can be saved [1][2].
Chronic Stability vs. Acute Crisis
Atherosclerosis can cause stable symptoms that are predictable and reproducible. For example, stable angina (chest discomfort) or intermittent claudication (leg cramping) typically occurs during physical exertion and is relieved by rest [3][4]. These symptoms should be managed with your doctor.
An acute crisis occurs when a plaque disrupts and a blood clot suddenly blocks an artery. This is not predictable, often happens at rest, and does not go away with usual measures [1][5]. Any new, worsening, prolonged, or different symptom needs urgent assessment even if it temporarily improves with rest or medication.
Heart Attack (Acute Myocardial Infarction)
A heart attack occurs when blood flow to the heart muscle is suddenly severely reduced or cut off.
Red Flags
- Sudden, severe chest pain, pressure, or “heaviness” that may spread to the arms, neck, jaw, or back [1].
- Chest discomfort that occurs at rest or is worsening in frequency or intensity [1][6].
- Sudden, unexplained shortness of breath [1].
- Feeling faint, lightheaded, or suddenly cold and sweaty (diaphoresis) [1].
Atypical Presentations
Not everyone has dramatic chest pain. Atypical symptoms are more common in women, older adults, and people with diabetes [7][8]. You should still seek emergency care for:
- Sudden, profound unexplained fatigue or weakness [1].
- Nausea, vomiting, or “indigestion” feelings that don’t match what you ate [1].
- Syncope (sudden fainting) [1].
Emergency Care: If a specific type of heart attack (STEMI) is confirmed by an ECG, hospitals aim to open the artery using primary PCI (stenting) within an eligibility goal of 120 minutes from medical contact [1][9]. Do not wait for symptoms to worsen toward this window—call emergency services immediately.
Stroke and TIA
A stroke occurs when blood flow to the brain is interrupted. A Transient Ischemic Attack (TIA), or “mini-stroke,” causes similar symptoms that resolve quickly, but it is a major warning sign—nearly 4% of people (depending on the population) have a full stroke within 7 days of a TIA [10][11].
BE-FAST Red Flags
Use the BE-FAST acronym to recognize a stroke:
- B - Balance: Sudden loss of balance or coordination [12].
- E - Eyes: Sudden vision loss or double vision [12].
- F - Face: One side of the face droops or is numb [2].
- A - Arms: One arm (or leg) becomes weak or numb, especially on one side of the body [2].
- S - Speech: Slurred speech or difficulty finding words [2].
- T - Time: Call your local emergency number immediately if any of these occur [2].
Emergency Care: For ischemic strokes, “clot-busting” medications (IV thrombolysis) are highly effective when given within eligibility windows of 3 to 4.5 hours [10]. Selected patients with large-vessel clots may require a thrombectomy to physically remove the clot, often within 6 to 24 hours depending on imaging [10][13]. Again, these are hospital goals, not deadlines you can safely wait toward.
Acute Limb Ischemia
This is a sudden, severe blockage of blood flow to a limb (usually a leg), requiring urgent surgery to avoid amputation [5][4].
The 6 P’s of Limb Emergency
Call emergency services immediately if a limb suddenly shows these signs:
- Pain: Severe, sudden pain even at rest [5].
- Pallor: The limb looks pale or blue [5].
- Pulselessness: You cannot feel a pulse in the foot or ankle [5].
- Poikilothermia: The limb feels significantly colder than the other [5].
- Paresthesia: “Pins and needles” or numbness [5].
- Paralysis: Inability to move the toes, fingers, or limb [5].
Emergency Care: Treatment involves immediate blood thinners like heparin and urgent surgery or catheter procedures to restore flow [14][15].
Urgent Bleeding
Because many cardiovascular patients take antiplatelet or anticoagulant medications, you must recognize signs of dangerous internal bleeding. Call for help immediately if you experience:
- Black, tarry stools or vomiting blood
- Sudden severe headache or neurologic change
- Severe, persistent bleeding that will not stop

Do not drive yourself or wait for a return call from your doctor. Emergency responders can begin life-saving treatments, perform an ECG, and notify the destination hospital immediately [1].
