Acute GVHD: The MAGIC Grading System
For acute GVHD, many centers use the MAGIC criteria (Mount Sinai Acute GVHD International Consortium). This system looks at the severity of symptoms in your skin, liver, and gut to assign a Stage (0–4) to each organ [1]. These individual stages are then combined into an overall Grade (I–IV) [2].
The Role of “Smart” Biomarkers (The MAP Score)
Doctors now have tools beyond just looking at a rash or counting bowel movements. They use a blood test called the MAP score (MAGIC Algorithm Probability) [1]. This test measures two specific proteins in your blood:
- ST2: A marker that indicates stress or damage to the heart and blood vessels [3].
- REG3\alpha: A marker that indicates damage specifically to the lining of the gut [1].
By combining these two markers, doctors get a much clearer picture of how “active” the GVHD is, even before physical symptoms get worse. A higher MAP score can tell your team that you might need more intensive treatment earlier to prevent complications [4][5].
Chronic GVHD: The NIH Consensus Criteria
Chronic GVHD is more complex because it can affect many different organs over a long time. To manage this, doctors use the NIH Consensus criteria [6].
Calculating Your Global Severity Score
Your doctor will look at eight different organ systems (Skin, Eyes, Mouth, GI tract, Liver, Lungs, Joints/Fascia, and Genital tract). Each is given a score from 0 to 3. These are then distilled into a Global Severity Score [6]:
- Mild: GVHD affects only one or two organs (excluding the lungs) with low severity [7].
- Moderate: Affects multiple organs moderately or includes mild lung involvement [7].
- Severe: Significant disability in any organ or major lung involvement [8].
Confirming the Diagnosis
Because chronic GVHD can look like other conditions, your team will use specific tests to “verify” what the score shows:
- Lungs: You will likely perform Pulmonary Function Tests (PFTs) to measure how much air you can breathe out [9].
- Eyes: An eye specialist might use Schirmer’s test (using a small strip of paper to measure tear production) or special dyes to look for surface damage [10][11].
- Tissues: If the diagnosis is unclear, a biopsy (taking a tiny tissue sample) may be used to confirm the presence of GVHD cells [12].
