1. The MIPI Score: Your Clinical Foundation
The Mantle Cell Lymphoma International Prognostic Index (MIPI) is a standard clinical tool used at the time of diagnosis to categorize patients into Low, Intermediate, or High-risk groups [1][2]. It is calculated using four specific variables:
- Age: Used because older age often affects how aggressively a patient can be treated.
- ECOG Performance Status: A 0-4 scale that measures your physical activity and ability to care for yourself.

- White Blood Cell (WBC) Count: The number of white cells circulating in your blood.

2. Ki-67: The Speedometer
The Ki-67 proliferation index is a percentage found on your biopsy pathology report. It tells you how many of your cancer cells are actively dividing at any given moment. Think of it as a “speedometer” for your lymphoma.
- The 30% Threshold: Most doctors consider a Ki-67 score of 30% or higher to be a critical threshold [5][3]. Scores above this level indicate a more aggressive, faster-growing disease.
- MIPI-c: Many specialists now use the MIPI-c (combined) index, which merges your clinical MIPI score with your Ki-67 percentage. This combined score is significantly better at identifying which patients may need more intensive therapy [6][5].
3. TP53: The Most Critical Molecular Marker
The TP53 gene is responsible for repairing damaged DNA or “ordering” a highly damaged cell to die. If this gene is mutated—or if the piece of the chromosome that holds it is missing (called a 17p deletion)—the cancer cell loses this “self-destruct” button.
This is incredibly important because it means the lymphoma becomes much harder to treat with traditional, DNA-damaging chemotherapy [7][8].
For patients with a TP53 mutation, standard intensive chemotherapy and an autologous stem cell transplant (ASCT) are often ineffective and unnecessarily toxic [7][9]. Current guidelines emphasize that TP53 status must be checked before starting treatment [10]. If a mutation is present, this is actually empowering information: it allows your doctor to pivot away from a stem cell transplant and move directly to targeted therapies (like BTK inhibitors) or clinical trials, which offer better outcomes than traditional high-dose chemotherapy [7][11].
