Understanding Your Baby's NICU Data

In the NICU, meconium aspiration syndrome (MAS) is monitored using chest X-rays, blood gases, and echocardiograms. The Oxygenation Index (OI) is a crucial metric that helps the care team track lung severity and adjust treatments like mechanical ventilation, inhaled nitric oxide, or ECMO.
In the NICU, you will be surrounded by monitors and data. While it may feel like a different language, these numbers and tests are the tools your baby’s care team uses to measure progress and adjust treatment. Understanding these tools can help you feel more informed and engaged in your baby’s care.
Common Diagnostic Tools for MAS
- Chest X-ray: This is often the first test performed. In a baby with MAS, the X-ray typically shows a “patchy” or “streaky” appearance, which represents areas where the meconium is blocking the airways [1]. You might also see “hyperinflation,” which means the lungs look very large on the screen because the mechanical obstruction has trapped air inside them [2].
- Blood Gases: Small amounts of blood are taken regularly to measure pH (acidity), pO2 (oxygen level), and pCO2 (carbon dioxide level). These numbers tell doctors exactly how well the lungs are moving oxygen into the blood and clearing out waste gases [1].
- Echocardiogram (Echo): This is an ultrasound of the heart. It is crucial for babies with MAS because it checks for Persistent Pulmonary Hypertension of the Newborn (PPHN) [3]. The Echo allows doctors to see if the blood pressure in the lung’s vessels is too high and if blood is being “shunted” or pushed away from the lungs [4].
- Lactate Levels: Lactate is a marker found in the blood. High or rising lactate levels can indicate that the body’s tissues are not getting enough oxygen [5]. As your baby improves, you will typically see these lactate levels begin to fall [6].
The Oxygenation Index (OI)
The Oxygenation Index (OI) is one of the most important numbers in the NICU for a baby with MAS. It is a calculation that combines the amount of oxygen your baby is receiving with the amount of pressure the ventilator is using [7].
- Tracking Severity: A higher OI generally means the lung disease is more severe [8].
- Guiding Treatment: Doctors watch the “trend” of the OI. If the OI stays high or rises, they may decide to escalate treatment, such as starting inhaled nitric oxide (iNO) or considering ECMO (a heart-lung bypass machine) [9][10].
A NICU Glossary: Terms You Will Hear
During rounds, you may hear technical terms used by the medical team:
- Atelectasis: Areas of the lung where the tiny air sacs have collapsed [1].
- Extubation: Removing the breathing tube when a baby is ready to breathe more on their own [11].
- FiO2: The “Fraction of Inspired Oxygen,” or the percentage of oxygen the ventilator is giving your baby (room air is 21%) [12].
- Inhaled Nitric Oxide (iNO): A gas breathed in through the ventilator that helps relax the blood vessels in the lungs to treat PPHN [13].
- Saturations (“Sats”): The percentage of oxygen in your baby’s blood, measured by the red light on their toe or hand [14].
- Weaning: The process of gradually decreasing the amount of oxygen or ventilator support as the baby gets stronger [15].

- Is the Oxygenation Index trending down today?
- What was the latest pH and lactate level?
- Are we weaning any ventilator settings (pressure or oxygen) today?
- Do we need a follow-up Echocardiogram to check the lung pressures?

- How is my baby’s pain, sedation, and overall comfort being managed while they are on the ventilator?
