The Consequences of Hemolysis

  • Dark Urine (Hemoglobinuria): When the level of free hemoglobin in your blood gets high enough, your kidneys try to filter it out. This turns your urine a dark color—often described as looking like tea, cola, or dark red wine [3][4].
  • Kidney Stress: Over time, this free hemoglobin can be toxic to the kidneys, leading to chronic kidney disease or sudden kidney injury if a major “flare” of hemolysis occurs [5][6].
  • Iron Deficiency: Chronic loss of hemoglobin in the urine also leads to severe iron loss (a condition called hemosiderinuria). This chronic iron deficiency is a massive contributor to fatigue in PNH and often requires proactive monitoring and iron supplementation [7].

Smooth Muscle Spasms and Nitric Oxide

One of the most unique aspects of PNH is how it affects your muscles. Normally, your body uses a gas called nitric oxide (NO) to keep your smooth muscles (the muscles in your digestive tract, blood vessels, and other organs) relaxed [8][9].

Free hemoglobin acts like a biological “sponge” for nitric oxide, soaking it up and leaving your body in a state of NO deficiency [8][10]. Without enough nitric oxide, your smooth muscles can go into painful spasms, leading to:

  • Abdominal Pain: Sudden, severe cramping in the stomach area [3][11].
  • Difficulty Swallowing (Dysphagia): Spasms in the esophagus that make it feel like food is getting stuck [12][11].
  • Erectile Dysfunction: Caused by the inability of blood vessels to relax properly [11][13].
  • Shortness of Breath: Sometimes caused by pulmonary hypertension (high blood pressure in the lungs) as the blood vessels there tighten up [14][9].

Fatigue: More Than Just Being “Tired”

Nearly every patient with PNH experiences fatigue, but it is often much more severe than the fatigue seen in other types of anemia [15][16]. Patients often describe it as a “crushing” or “debilitating” exhaustion that doesn’t go away with rest. While low red blood cell counts play a role, the ongoing process of hemolysis, iron loss, and the loss of nitric oxide are major contributors to this systemic drain on your energy [17][7].

Thrombosis: The Most Serious Complication

Historically, thrombosis (blood clots) has been the leading cause of death for people with untreated PNH [18][15]. PNH creates a “perfect storm” for clotting because the immune system activation and the cell debris from hemolysis make the blood much stickier than normal [19][2].
