Diagnosing pediatric systemic lupus erythematosus (pSLE) is rarely a straightforward process. Because the disease often begins with mild, nonspecific symptoms, there is frequently a delay between the first signs of illness and a formal diagnosis [1]. To help doctors navigate this complexity, they use standardized sets of “rules” called classification criteria.
Understanding Diagnostic Criteria
Doctors primarily use two sets of criteria to determine if a child has lupus. These were originally designed for adults, but they are highly effective for children as well:
- SLICC 2012 Criteria: This set is known for its high sensitivity, meaning it is very good at “catching” cases of lupus even when symptoms are early or atypical [2].
- EULAR/ACR 2019 Criteria: These are the most current guidelines and are considered the most appropriate for pSLE today [3]. They require a positive ANA (antinuclear antibody) test as an “entry” point and then use a weighted point system to balance accuracy (not missing a case) with precision (not misdiagnosing another condition) [4][5].
The Role of Genetic Testing
While most lupus cases are caused by a mix of genetics and environment, some pediatric cases are monogenic—driven primarily by a single gene mutation. Genetic testing is particularly valuable in specific scenarios [6]:
- Very Early-Onset: Children diagnosed before age 5 have a much higher likelihood of an underlying genetic cause [1][6].
- Atypical Presentations: When a child has symptoms that don’t fit the usual pattern or has a strong family history of early-onset autoimmunity.
- Specific Mutations: Testing can identify deficiencies in proteins like C1Q or mutations in genes like UNC93B1 [7]. In children with UNC93B1 variants, the immune system becomes “hyper-responsive,” reacting too strongly to signals that it should ignore [8][9].

Evaluating Kidney Health: The Renal Biopsy
One of the most critical steps in the pSLE work-up is checking the health of the kidneys, as 50% to 80% of children with lupus develop kidney involvement [10].
Doctors will carefully monitor blood and urine tests. They look closely at complement proteins (C3 or C4) in the blood, as low levels can be an early clue of “silent” inflammation happening in the organs [11].
If clinical indicators suggest kidney involvement (such as protein in the urine, active urine sediment, or a rising creatinine level), a renal biopsy becomes essential to determine the type and severity of inflammation [11]. This procedure involves taking a tiny sample of kidney tissue, typically performed with a small needle under ultrasound guidance while the child is sedated or asleep, ensuring they do not feel pain.
Why Diagnosis Can Be Delayed
For the majority of children, pSLE has a “mild” start [1]. Because the symptoms—like fatigue, growing pains, or a faint rash—can look like many other childhood issues, it often takes months to reach the right specialist. Only a small number of children present with severe, multi-organ disease from day one that leads to a rapid diagnosis [1].
