The Lab Work: Checking for Destruction
Before a doctor can confirm wAIHA, they must first prove that hemolysis (the premature destruction of red blood cells) is actually happening [1]. They use several standard tests:
- Complete Blood Count (CBC): This measures your hemoglobin level. A low number confirms you have anemia [1].

The Direct Antiglobulin Test (DAT)
The Direct Antiglobulin Test (also called the Coombs Test) is the most critical tool for diagnosing wAIHA [7]. It looks for specific proteins stuck to the surface of your red blood cells.
What do the results mean?
A laboratory will test your blood for two main markers:
- IgG (Immunoglobulin G): This is the antibody itself. If your test is positive for IgG, it means your immune system has “tagged” your blood cells for destruction [1][8].
- C3d (Complement): This is part of a system that helps antibodies do their job. If you are positive for both IgG and C3d, your case may be more aggressive [9].
When the Test is Negative (DAT-Negative wAIHA)
In about 5% to 10% of cases, a patient has all the symptoms of wAIHA, but the standard DAT comes back negative [10]. This is called DAT-negative AIHA.
This usually happens because the amount of antibody on the cells is too small for standard tests to see, or because the antibodies are “low-affinity” and wash off during the testing process [10][11].
