Because Lyme disease requires antibiotics and JIA is managed with anti-inflammatory or immunosuppressive therapies, doctors must definitively rule out an active Lyme infection before confirming a JIA diagnosis [3].

Surprisingly, despite how swollen the joint looks, individuals with Lyme arthritis often experience relatively little pain and may still walk, run, or play with minimal distress.
- Morning Stiffness (JIA): JIA tends to develop more gradually. A hallmark characteristic of JIA is prominent morning stiffness (often called the “gel phenomenon”) [2]. People with JIA frequently wake up stiff, may limp, or show reluctance to use the affected joint first thing in the morning. This stiffness usually improves as the day goes on and the joint is moved around. While Lyme often targets the knee, JIA frequently impacts smaller joints like ankles or wrists as well.
The Definitive Blood Tests
Because symptoms alone are not enough to confirm a diagnosis, doctors rely on specific laboratory tests [1][2]. Pediatric rheumatologists will use a highly reliable “two-tier” testing method to check for Lyme disease [5]:
- Lyme Titer (EIA/ELISA): This is the initial screening blood test. It checks for antibodies (proteins your immune system makes to fight off infections) against the bacteria that causes Lyme disease. If this test is negative, it typically means Lyme disease is not the cause of the swelling [5][6].
- Confirmatory Test: If the initial screening test is positive or unclear, doctors perform a second confirmatory test. Historically, this has always been a Western blot, but many modern labs now use a second, specialized EIA/ELISA test based on updated protocols [7][8]. Both methods look for particular antibody patterns to verify an active or past Lyme infection.
These blood test results usually take a few days to a week to come back. In some ambiguous cases, a doctor may also test synovial fluid (the liquid inside the joint) to help rule out other types of infections [9].

Note: If a patient is diagnosed with Lyme disease, treated successfully with antibiotics, but joint swelling persists for weeks, this is known as antibiotic-refractory Lyme arthritis [10][11]. While it may be treated with anti-inflammatory medications similar to those used for JIA, it is a distinct condition, not JIA.
