Juvenile idiopathic arthritis (JIA) is not directly hereditary in the way that eye color or height is passed down. While having a parent with an autoimmune disease does mean a child might inherit a slightly higher genetic susceptibility, you did not directly “pass down” JIA to your child. JIA is what doctors call a complex, polygenic condition [1]. This means it takes a combination of many different genes, plus an unpredictable, everyday environmental trigger, to cause the illness [2][3][4]. You did not cause this, and there is nothing you could have done to prevent it.
The Role of Genetics in JIA
Genetics do play a part, but they only set the stage. Instead of a single “arthritis gene,” research shows that JIA involves many different genes interacting together [5][6]. Some of these genes are in the HLA (Human Leukocyte Antigen) region, which is a group of genes that helps control how the immune system behaves [1][7]. Because you have an autoimmune condition yourself, it is possible you and your child share some of these immune-regulating genes. This shared susceptibility is why we sometimes see various autoimmune conditions cluster in families [8][9].
However, genetics are not destiny. Even identical twins—who share 100% of their DNA—only both develop JIA about 25% to 40% of the time [8]. This proves that genetics alone are simply not enough to cause the disease.
Environmental Triggers: The Missing Puzzle Piece
Because genes only create a susceptibility (a tendency to develop the condition), a child also needs to encounter an environmental trigger that essentially “turns on” the disease [1][10]. These triggers are common, everyday exposures that affect the immune system. Studies have identified several potential triggers:
- Infections: Common childhood viruses (such as Parvovirus B19 or Epstein-Barr virus) can sometimes confuse the immune system and trigger autoimmune responses [11].
- The Gut Microbiome: The balance of healthy bacteria in the digestive tract plays a significant role in training the immune system. Disruptions to this balance, known as microbial dysbiosis, have been linked to JIA [12][13]. While this is an active area of research, there is currently no evidence that a parent’s dietary choices caused this imbalance, or that specific diets and probiotic supplements can cure JIA.
- Antibiotics: Because antibiotics affect gut bacteria, early exposure to them has been statistically associated with a slightly higher risk of developing JIA [14]. However, antibiotics are life-saving and medically necessary for bacterial infections. Treating your child’s past ear or throat infections was the absolute right thing to do, and you should never feel guilty for following medical advice. Furthermore, you should not withhold necessary antibiotics for future illnesses based on a JIA diagnosis.
It is important to remember that these everyday triggers are believed to be the “match” that starts the initial illness. You do not need to obsessively shield your child from the normal world or feel paralyzed about how to handle common childhood illnesses moving forward.
Are There Exceptions?
True JIA is a complex mix of genetics and environment. However, there are some very rare genetic conditions—called autoinflammatory syndromes (such as familial Mediterranean fever)—that can look almost exactly like the systemic form of JIA [15][16]. Unlike JIA, these rare syndromes are often caused by a single gene mutation and can be directly passed down in families [16]. If your child has recurring, unexplained fevers alongside joint issues, doctors may test for these rare conditions to rule them out.
