The Recipe for Hemoglobin
To understand why the adult test looks normal, it helps to know how hemoglobin is made. Normal adult blood contains three types of hemoglobin, and they all rely on alpha-globin chains [3]:

Because alpha-thalassemia trait involves a shortage of alpha chains, the production of HbA, HbA2, and HbF are all slowed down. The overall amount of hemoglobin is smaller, but the proportions of each type remain in the normal range. In fact, HbA2 might even be slightly lower than average because the body’s beta chains outcompete the delta chains for the limited supply of alpha chains.
The Beta-Thalassemia Contrast
Many people with alpha-thalassemia trait are confused by normal test results because they know someone with beta-thalassemia trait whose condition was easily diagnosed with a standard blood test.
In beta-thalassemia, the body lacks beta chains. To compensate for this missing ingredient, the body ramps up production of the hemoglobins that don’t need beta chains—specifically Hemoglobin A2 (HbA2) [4].
