Understanding Symptoms and Biology of HSV Stromal Keratitis

Herpes Simplex Virus (HSV) stromal keratitis is a complex condition where the eye’s immune system overreacts to the presence of the virus. While other forms of herpes eye infections involve the virus attacking the surface of the eye, stromal keratitis is primarily an inflammatory disease occurring in the deeper layers of the cornea (the clear front window of the eye) [1][2].

How Stromal Keratitis Differs from Surface Infections

Most people are familiar with epithelial keratitis, which is a surface infection. In that stage, the virus is actively replicating on the surface, creating visible, branch-like patterns called dendrites. This usually causes sharp, gritty pain and redness.

In contrast, stromal keratitis happens deeper within the structural layer of the cornea. Because it is deeper, the symptoms and the biology of the disease are different:

  • Hazy Vision: Instead of a “scratchy” feeling, you may notice that your vision is cloudy or hazy. This is caused by edema (swelling) or infiltrates (white blood cells) clouding the normally clear cornea [1][3].
  • Deep Ache: The pain may be more of a dull, deep ache rather than the sharp sensation of a surface scratch.
  • Light Sensitivity: You may find bright lights significantly more uncomfortable than usual.
  • Invisible Damage: Unlike surface lesions, the damage in the stroma can occur even if the surface of the eye looks smooth to the naked eye. Doctors use a slit-lamp (a specialized microscope) to see the inflammation [1][4].

The Biology: An Immune “Friendly Fire”

The most important thing to understand about stromal keratitis is that it is often an “immune-mediated” disease. This means your own immune system is causing the damage while trying to find the virus [5][6].

The process is driven by CD4+ T-cells, which are the “commander” cells of your immune system. When the virus reactivates from its hiding place in the nerves, it leaves behind viral proteins (antigens) in the corneal stroma. Even if the virus is no longer actively replicating, your T-cells detect these leftovers and mount a massive attack [7][8].

This “friendly fire” results in:

  • Infiltration: Thousands of white blood cells flood the cornea, sometimes forming a visible circle of inflammation known as a Wessely ring [9][10].
  • Scarring: If the inflammation lasts too long, the orderly structure of the cornea is permanently disrupted, leading to scars that can block vision [5][3].
  • Neovascularization: The body may try to heal the area by growing new blood vessels into the cornea. Since the cornea must be clear to see, these vessels can cause further vision loss [5].

Why the Eye Loses Sensation

A hallmark of HSV keratitis is corneal hypesthesia, or a loss of feeling in the eye [11]. This happens because the Herpes Simplex Virus lives in the trigeminal ganglion, a cluster of nerve cells near your brain.

When the virus wakes up, it travels down the nerves like a highway to reach the eye [12][13]. During this journey and the subsequent immune battle, the delicate sub-basal nerve plexus (the network of nerves just under the eye’s surface) is damaged or destroyed [11][14].

This loss of sensation is dangerous because:

  1. Reduced Warning: You may not feel a new scratch or infection, allowing it to become severe before you notice.
  2. Poor Healing: The nerves actually release chemicals that help the surface of the eye heal. Without these signals, the eye’s surface can become fragile and break down [15].
